Who Needs a BiPAP Machine?
BiPAP therapy is prescribed for a specific set of respiratory conditions — always under the guidance of a pulmonologist or sleep physician. Here are the conditions where BiPAP is commonly considered.
COPD
BiPAP can assist ventilation and help manage chronic CO₂ retention in COPD patients, under physician guidance.
Learn MoreSleep Apnea
Prescribed when CPAP alone hasn't resolved symptoms, or for central and complex sleep apnea patterns.
Learn MoreObesity Hypoventilation Syndrome
BiPAP's dual pressures can assist ventilation in OHS patients with daytime CO₂ retention.
Learn MoreNeuromuscular Disorders
A backup breath rate provides a safety margin for patients with reduced respiratory drive.
Learn MoreCan COPD Patients Use a BiPAP Machine?
What is COPD?
Chronic Obstructive Pulmonary Disease (COPD) is a group of lung conditions — including emphysema and chronic bronchitis — that progressively narrow the airways, making it harder to breathe out fully. Over time, this can lead to chronic CO₂ retention (hypoventilation) in some patients.
Why is a BiPAP machine for COPD recommended?
When a pulmonologist identifies chronic hypoventilation, a BiPAP machine for COPD can help — its two independent pressures (IPAP/EPAP) assist ventilation and can support easier exhalation, which a single-pressure device does not address.
BiPAP and COPD: how is it different from oxygen therapy?
Oxygen therapy raises blood oxygen levels but does not assist the mechanical work of breathing. BiPAP for COPD does the opposite job — it supports ventilation and can help the body clear CO₂ — and the two are sometimes used together as prescribed by a physician.
Who should use BiPAP for COPD?
- COPD patients with confirmed chronic CO₂ retention
- Those for whom standard CPAP or oxygen alone hasn't been sufficient
- Patients prescribed BiLevel-ST therapy by a pulmonologist
- Patients continuing hospital-started BiPAP therapy at home
This is general information, not medical advice. Whether a BiPAP machine for COPD is right for you should always be confirmed by a pulmonologist based on your diagnosis.
Read more about BiPAP for COPDWhat is BiPAP therapy and who needs it?
BiPAP (Bilevel Positive Airway Pressure) is a form of non-invasive ventilation (NIV) that delivers two pressure levels — a higher pressure when you inhale (IPAP) and a lower pressure when you exhale (EPAP). This makes it significantly easier to breathe than single-pressure CPAP at high settings.
BiPAP is prescribed when CPAP therapy is not sufficient, or for conditions beyond OSA that involve respiratory muscle weakness or impaired gas exchange.
Who is prescribed BiPAP?
- Severe OSA not responsive to CPAP
- COPD with hypercapnia (high CO₂)
- Congestive heart failure (CHF)
- Obesity hypoventilation syndrome (OHS)
- Neuromuscular disease (ALS, MD)
- Post-surgical respiratory support
Impaired breathing
Weakened or obstructed airway causes restricted airflow, elevated CO₂, poor oxygen delivery, and disrupted sleep.
Supported breathing
BiPAP delivers two pressures — higher on inhalation, lower on exhalation — keeping the airway open and normalising CO₂.
Types of BiPAP Machines
BiPAP S (Spontaneous)
Cycles on your own breathing effort. First-line for high-pressure OSA and mild-moderate COPD.
BiPAP ST (with Backup Rate)
Adds a timed backup breath if you stop breathing. Prescribed for central apnea, NMD, COPD with hypercapnia.
BiPAP AVAPS
Auto-adjusts to deliver a target tidal volume. Used for COPD, OHS and complex respiratory failure.
How does a BiPAP machine work?
Higher pressure on inhalation (IPAP)
When you breathe in, the machine delivers a higher pressure to push air into the lungs — opening the airway and ensuring adequate oxygen delivery.
Lower pressure on exhalation (EPAP)
When you breathe out, pressure drops — making it much easier to exhale. This is the key advantage over CPAP for patients with higher pressure requirements.
Backup rate (in BiPAP-ST)
If you stop breathing, the machine delivers a timed backup breath — essential for central apnea, NMD, and post-surgical patients.
Better sleep and gas exchange
Normalised CO₂ levels, improved oxygen saturation, reduced work of breathing, and restful sleep — even for complex respiratory conditions.

5 Things to Check
Get a prescription first
BiPAP settings (IPAP, EPAP, backup rate) must be prescribed by a pulmonologist or sleep physician. Do not self-prescribe.
Choose the right mode (S / ST / AVAPS)
BiPAP-S for OSA, BiPAP-ST for central apnea or NMD, AVAPS for COPD or OHS. Your doctor prescribes the mode.
Include a heated humidifier
BiPAP at higher pressures causes significant nasal dryness. A heated humidifier dramatically improves comfort and adherence.
Verify data monitoring
Compliance and residual AHI data lets your doctor optimise therapy remotely. Löwenstein uses prisma APP and CLOUD.
Buy from an authorised dealer
