How to choose a BiPAP machine

A BiPAP machine gives two pressures: a higher one when you breathe in (IPAP) and a lower one when you breathe out (EPAP). Beyond that, three things decide which machine is right for you, and all three come from your doctor's prescription.

1. Does it need a backup breath?

An S/T or T mode gives a breath on its own if you pause for too long. Patients with COPD, obesity hypoventilation or weak breathing muscles usually need it. Both the prisma 25ST and the prisma 30ST have it.

2. How high is the prescribed pressure?

The prisma 25ST reaches an IPAP of 25 cmH₂O, which covers most sleep apnea and many COPD prescriptions. If your doctor sets pressures above that, or wants home NIV with a tightly controlled backup rate, the prisma 30ST goes up to 30 cmH₂O.

3. Is the apnea central or complex?

When the brain does not send a steady signal to breathe, ordinary bilevel therapy may not be enough. A sleep specialist may then prescribe adaptive servo-ventilation (ASV), which is what the prismaCR does.

Not sure yet? Bring your sleep study or blood gas report and we will go through it with you, or start with a sleep apnea test.

Best BiPAP Machine: Frequently Asked Questions

There is no single best BiPAP for everyone; the right one depends on your prescription. For most home BiPAP prescriptions we suggest the Löwenstein prisma 25ST, which covers CPAP, S, S/T and T modes. If your doctor prescribes pressures above 25 cmH₂O or home NIV, the prisma 30ST is the better fit. For central or complex sleep apnea, a specialist may prescribe the prismaCR (ASV).
